Reluctance to Learn

My son is 3 1/2 years old, he speaks very little words and most of the time you cannot understand him.

He has no other illnesses and I did him an IQ test and it was goof, his teacher in nursery understands him through gestures and funny words.

The problem he is not willing to try to speak and is not responding to the speech doctor.

What can I do?
