Is it shyness or speech problems?

My son has problems with having words come out he knows what to say but can't seem to push them out he has to pound on his chest or stomp on the floor to get them out. He cannot hold long conversations because he struggles with a continuous pounding of the floor to get his words out he reads out loud but still has the same issue he says the words don't want to come out. School therapist said hes probably just shy. He cries when he has to talk in front of his class because he hates having to hit himself in order for the word to come out. Any suggestions on what I can do for him or if it's shyness or speech issues?
