Articulation issue?

Hi!
I have a four year old client (turning five soon) who is very anxious. He tends to mumble his speech in a low tone of voice. He does have some speech errors: gliding of the /l/ phoneme and substitutes the medial /d/ phoneme for an /l/. Aside from the above deficits, he tends to drop consonants during connected speech. At times it is difficult to understand him. How would you target this? What I have been doing has not been working so well!!
