Medicare and Medicaid Documentation

Hi all,

I'm a speech path with lots of school experience who has recently moved to the home health/outpatient setting.

Can anyone direct me to some good resources and examples of the type of documentation I need to complete here (evaluations, SOAP notes, discharges, etc).

Any help and guidance would be very appreciated!
